Position title: Case Manager – Rapid Response Program

Organisation:    The Haymarket Foundation Ltd.

Grading: SCHCADS Level 4.

Employment Type: Permanent – Full time

 

We are looking for a positive, energetic, and adaptable individual to join us as a Case Manager in our outreach program, Rapid Response.  In this role you will be part of a small team who work closely, so we are looking for someone who loves working in a team and has great communication skills. The role will support clients experiencing homelessness to establish sustainable housing pathways whilst delivering holistic and responsive case management.

About the role

The role of the Rapid Response case manager is to support people over the age of 18 who are either at imminent risk of homelessness or who have recently become homeless. A major component of the role is supporting clients in our outreach properties, through adaptive case management whilst focusing on long-term client directed outcomes.

External relationship building across external networks, relevant accommodation services/Housing NSW/boarding houses is also critical to the role. The successful candidate will be supported through external supervision, regular team meetings and staff development activities.

This position is Monday to Friday and reports to the Manager: Homelessness Programs and will work closely with other staff members. The program is located in Chippendale and has a salary and conditions as per Grade 4 of the SCHCADS Award.

Position title:    Case Manager: HIV & AOD Integrated Care Program

Organisation:    The Haymarket Foundation Ltd.

 

We are currently seeking a motivated, adaptable and experienced case manager who is able to demonstrate leadership within a challenging residential setting.  The role is suited to someone who has experience in case management for clients with complex co-occurring disorders and possesses the ability to multitask in a fast-paced environment. The candidate will be client focused, with an ability to implement adaptive case management. We focus on providing support, stabilisation, and advocacy to establish safe housing pathways for our clients with a goal of long-term, sustainable and independent living.

The HIV & AOD Integrated Care Program is a 4-bed intensive unit located within the Haymarket Foundation’s Residential Services for women, men and transgender people over the age of 18 with a focus on access and equity for people from all backgrounds. The program focuses on providing stabilisation and housing pathways for clients, and also offers a number of residential services, including meals (breakfast, lunch, and dinner) showers, and laundry facilities.

Position Information:  This is a fulltime role, working on a monthly roster (morning and afternoon shifts) including weekends. The position reports to the Manager of Homelessness Services and will work closely with other residential services staff. The program is located in Chippendale and has a salary and conditions to per Grade 4 of the SCHADS Award (including applicable loadings).
